In 11 CAR T cell patients with ICANS (grade 1: n=3; grade 2: n=4; grade 3: n=1; grade 4: n=3), the authors combined scRNA-seq, paired blood/CSF flow cytometry, ELISA, and spatial transcriptomics/immunofluorescence on one fatal grade-4 postmortem brain . Flow cytometry showed CAR+CXCR6+ CD4+ T cells were significantly enriched in CSF versus paired blood, while CD8 counterparts were not β a compartment-specific signal. scRNA-seq revealed CXCR6 upregulation co-occurring with GZMA expression, cell-cycle (G2M/E2F) enrichment, and clonally diverse CD4+ expansion rather than a single dominant clone.
Spatially, CXCR6+CD3+ T cells in periventricular parenchyma sat within 100 Β΅m proximity of CXCL16+CD11b+ myeloid cells at higher ratios than near CXCL16β myeloid cells, and these myeloid cells co-expressed IL-18 and Ki-67+ T cells indicated proliferation β consistent with an infiltrating (CD40/CD49d/ITGAL+) monocyte-derived CXCL16 source rather than resident microglia .
The axis is not novel per se: mouse WNV-recovery models show CXCL16/CXCR6 maintains CNS-resident CD8+ TRM cells driving microglial activation and synapse loss, with genetic and antibody-based necessity proof , and amyloid models implicate myeloid CXCL16 in CXCR6+ CD8+ T cell recruitment . What is genuinely new is the human CSF/spatial confirmation in an iatrogenic neurotoxicity setting β and the absence of comparable human necessity data (no CXCR6 blockade trial exists).
No CAR-T-without-ICANS CSF control: the authors explicitly acknowledge this ethical constraint; controls are IIH, FND, and MS patients from 2011β2024, so CXCR6 enrichment could partly reflect generic neuroinflammation rather than ICANS-specific biology. Confounded CSF CXCL16: CSF CXCL16 correlated with CSF IL-6 (Fig. 2G), weakening claims of axis-specificity over general CNS inflammation. Temporal inference from one patient: the longitudinal pre-infusionβonsetβremission trajectory supporting CXCR6 dynamics mirrors-disease rests on pt. 1 alone β a HARKing-adjacent risk. Postmortem n=1, four ROIs: proximity statistics (paired t-test across four 1000Γ1000 Β΅m regions) treat ROIs as replicates from a single brain, risking pseudoreplication. Conflict of interest: four authors hold a patent application on diagnostic/therapeutic use of the CXCL16/CXCR6 axis in ICANS, a direct financial incentive toward positive framing . Data are deposited (GEO GSE269379), supporting reproducibility.
Prospective CSF sampling of CAR-T patients without ICANS showing equivalent CXCL16/CXCR6 signals would refute disease-specificity; a prospective multi-cohort test showing CSF CXCL16 loses severity-correlation after IL-6 adjustment would collapse the biomarker claim; and an intervention blocking CXCR6 recruitment without altering ICANS would sever the mechanistic link.
