Intensity-controlled treadmill exercise (LI 55-60% VO2 max vs HI 85-90% VO2 max) in adult male Wistar rats (~230g) over 4 weeks produced measurable structural remodeling: LV mass increased from 848.0 Β± 44.9 mg (control) to 1097.3 Β± 44.5 mg (HI, P=0.002), and total ventricular cardiomyocytes rose 4% (LI) and 7% (HI).
Beyond hypertrophy, c-kit+ endogenous cardiac stem-progenitor cells (eCSCs) expanded two-fold at 2 weeks and remained elevated at 4 weeks; baseline frequency was ~1 eCSC per 1500 myocytes, with Ki67+ cycling eCSCs at 27 Β± 4% (2 weeks) and 16 Β± 4% (4 weeks). At least 60% of cycling eCSCs expressed Nkx2.5 (myocyte) or Ets-1 (endothelial) lineage markers, indicating active commitment. Small BrdU+ mononucleated cardiomyocytes (7.4 Β± 0.3% after 4 weeks HI) suggest newly formed myocytes, and BrdU+ vWF+ capillaries were detected alongside increased capillary density.
Cardiomyocytes upregulated IGF-1 and TGF-Ξ²1 at 7 and 14 days, with BMP-10, NRG-1, and periostin upregulation confirmed by Western blot. In vitro, IGF-1 and NRG-1 increased eCSC BrdU incorporation 5Γ and 3Γ (additive), activating Akt and STAT3; BMP-10 drove 3Γ endothelial and 5Γ SMA+ differentiation, while TGF-Ξ²1 drove 3Γ cTnI+ and 9Γ SMA+ differentiation via Smad2 phosphorylation. Mature cardiomyocytes showed no BrdU incorporation under these growth factor treatments, suggesting effects act on progenitors rather than existing myocytes.
BGPT inference (not author claims): the evidence is largely correlative β no definitive in vivo lineage-tracing proves the precursor-product relationship between eCSCs and new myocytes. BrdU labeling cannot distinguish cell-cycle re-entry from DNA repair. Sample sizes were small (nβ6 per group per time point), results are rat-specific with uncertain human extrapolation, and long-term post-detraining fate was not assessed. The c-kit+ cardiac stem cell field itself remains contested, with ongoing debate over whether c-kit+ cells generate cardiomyocytes at meaningful rates in vivo, so these findings should be interpreted as suggestive of an endogenous regenerative component rather than conclusive proof of stem-cell-driven myogenesis.
