The CoVPN 3008 study (NCT05168813) enrolled 12,480 adults across seven East/Southern African countries, 83% with HIV, comparing hybrid immunity (prior infection + one mRNA-1273 dose, N=9613) vs. vaccine-only immunity (two doses, N=2867). Using case-cohort sampling with MSD-ECL binding assays and VSV-pseudovirus neutralization, 206 Serum-IAS endpoints (152 Hybrid, 54 Vaccine) were analyzed
Hybrid Group (N=287, 195 PWH): M0 nAb-ID50 BA.4/5 was the only independent inverse correlate through 230 days post-Peak (HR=0.46; 95% CI 0.28β0.75; P=0.002), robust to adjustment for IgG Spike BA.4/5 and IgG N Index (Table 2, Models 2β3). Peak markers did not correlate (Peak nAb HR=1.13, P=0.45)
Vaccine Group (N=115, 86 PWH): Peak nAb-ID50 BA.4/5 inversely correlated through 92 days (P=0.015) but not 230 days (P=0.959); controlled risk fell from 1.9% (titer 10 AU/ml) to 0.3% (1000 AU/ml)
PWH vs. PWoH: No multiplicity-adjusted evidence of HIV effect modification; Peak responses in the Vaccine Group were unexpectedly higher in PWH (nAb difference 0.48-log, 95% CI 0.17β0.81), which the authors attribute to unmeasured confounding
Strengths: Rigorous case-cohort design with inverse probability weighting, Holm-Bonferroni correction, cross-trial calibration to COVE/COVAIL via assay concordance studies (adjusted RΒ² 0.91), and public R code (GitHub, Zenodo)
Limitations the paper acknowledges: No placebo arm, precluding causal CoP inference; no control for breakthrough lineage; PWH were predominantly well-controlled (94% CD4 β₯200), limiting generalizability to advanced HIV
Additional BGPT observations: The Vaccine Group is small (N=115; 54 cases), yielding wide CIs (e.g., Peak IgG Spike HR 0.56, CI 0.14β2.21) that limit subgroup power; all endpoint detection was symptom-triggered without scheduled nasal swabbing, which may misclassify asymptomatic infection and shift case timing. The flat Peak-risk curve in the Hybrid Group contrasts with M0 significanceβauthors infer fold-rise mediation, but this inference is indirect and unmeasured confounding (e.g., pre-existing T-cell memory) remains plausible. The authors' interpretation that correlates were "qualitatively and quantitatively similar" across HIV status rests on interaction tests that were powered for only the largest contrasts; smaller true differences cannot be excluded.
What would change the conclusion: A placebo-arm trial with scheduled NAAT testing showing M0 nAb titers no longer associate with infection risk among hybrid-immune individuals after accounting for breakthrough lineage and timing, or replication in PWH with uncontrolled viremia (CD4 <200), would falsify the reported correlate claims.
