This preprint provides the first direct mechanistic evidence that EBNA1-primed CD4+ T cells can target ANO2, an MS-associated autoantigen. Using 27,524 single-cell TCR sequences from four natalizumab-treated pwMS, the authors found 55 expanded clonotypes cross-reactive between EBNA1 and ANO2, with 43 tri-cross-reactive clonotypes spanning EBNA1, ANO2, and CRYAB .
ANO279-168-specific IFNΞ³+ T cells were elevated in untreated and natalizumab-treated pwMS, with ~57% of untreated pwMS responding (vs ~15% previously reported for anti-ANO2 antibodies). ROC AUCs reached 0.77β0.85 across cytokines . The antibody cohort used the Swedish EIMS (713 pwMS, 722 controls), confirming prior EBNA1-ANO2 antibody cross-reactivity findings . Contextually, other work confirms elevated EBNA1 IgG in MS and genetic associations , and independent single-cell work found EBV-reactive CD8+ T cells enriched in CSF .
ANO2 pre-immunisation worsened PLP-induced EAE in SJL/J mice, with older mice showing atypical phenotypes (descending paralysis, seizures, reduced survival) linked to cerebellar ANO2 expression in the vasculature (76% CD31 colocalization) .
Key limitations: single-cell TCR analysis used only n=4 natalizumab-treated donors; natalizumab skews peripheral T cell phenotypes; bead-based antigen presentation may miss native epitopes; EAE-to-MS extrapolation is untested in humans; TCR overlap alone does not prove in vivo cross-reactive pathogenicity. The paper addresses these by acknowledging natalizumab confounds and comparing TCRs with public datasets (20/55 cross-reactive clonotypes found in Amoriello/Gottlieb datasets), but no raw data are deposited with accession numbers, limiting reproducibility .
What would falsify this: showing ANO2 T cells are not elevated in untreated pwMS vs HC, that EBNA1/ANO2 TCR overlap disappears with paired TCRΞ±Ξ² chain analysis, or that ANO2 pre-immunisation does not exacerbate EAE when replicated.
