Population: Adults ≥60 y with fasting plasma TMAO above cohort median (~260 ng/mL in gallstone-free controls; gallstone patients averaged 398 ng/mL ); stratify by age band (60–74 vs ≥75) and sex; exclude antibiotic use in prior 3 months, active antiplatelet escalation, or FMO3 polymorphism screening optional. Randomization: 1:1, computer-generated permuted blocks within strata, blinded participants, investigators, lab analysts. Duration: 12 weeks; visits at 0, 4, 8, 12 weeks; blood drawn fasting at standardized morning slots.
Primary PD endpoint: Percent change in plasma TMAO (LC-MS/MS, stable-isotope dilution — the method validated in the Cell cohort ) and, for the Phe-pathway arm, plasma PAGln. Hypothesis: ≥30% reduction vs placebo.
Secondary endpoints: (1) Flow-mediated dilation (brachial, standardized stimulus); (2) ex vivo platelet aggregation to ADP/thrombin by light-transmission aggregometry — justified because physiological TMAO shifted ADP/thrombin activation curves leftward in donor platelets (p<0.0001 at 100 mM) ; (3) circulating p16INK4a+ cells and SASP cytokines (IL-6, IL-8, MMPs) — exploratory, since plasma p16 senescence quantification in this context is not directly validated by supplied evidence.
Analysis plan: Intention-to-treat mixed-effects models (change from baseline ~ treatment × time + strata, random intercept); multiplicity via Holm for the two primary metabolites; Spearman correlation between ΔTMAO and Δaggregation to test mechanism. Power: with n=110 evaluable/arm, detectable standardized effect size d≈0.54 at 80% power (α=0.05), adequate for a 30% TMAO drop given typical CV of ~35%.
Critical blind spots: Supplied evidence is association + murine/ex vivo mechanism only — no prior RCT demonstrates that inhibiting microbial TMA lyase lowers TMAO in humans; the murine FeCl₃ model and 100 mM in vitro doses may not reflect endogenous human levels . Confounders (dietary choline intake, kidney function) need collection. Falsification: no ΔTMAO or no dose-response relation between ΔTMAO and Δplatelet reactivity.
