Wojciechowska-Czarnik and Yang build their case on four converging observations: asthma is heritable yet genetics explains only a small fraction of its aetiology; maternal-line transmission exceeds paternal; asthma prevalence rose too fast for sequence change alone; and in-utero exposures (smoke, traffic pollution, diet) demonstrably alter methylation marks . The mechanistic core is T-cell lineage control: DNMT3A-mediated methylation of the Ifng promoter, GATA-3-driven demethylation of the Th2 locus control region, and FOXP3 methylation governing Treg stability.
The strongest causal evidence is murine. In the authors' own model, a high maternal methyl-donor diet (folic acid, B12, choline, betaine) increased pup airway inflammation, serum IgE and airway hyperresponsiveness, with 82 gene-associated CpG islands hypermethylated including the Runx3 promoter β and critically, the phenotype was reversed by 5-aza-deoxycytidine, a pharmacological reversal test that most epigenetic association studies never perform . Conditional DNMT3A deletion in CD4+ T cells similarly increased IL-13, airway inflammation and AHR β necessity evidence for the methylation machinery itself.
Human studies cited are all association designs in surrogate tissues (peripheral blood, buccal, nasal cells, sputum, cord blood) β never lung tissue. Sample sizes vary enormously, and several are underpowered pilots:
The review itself concedes these studies "have only demonstrated statistical association" without elucidating methylation's role in gene expression control in human asthma. Two notable exceptions partially escape this limit: the Alox12 hypomethylationβpersistent wheeze link replicated across two independent Spanish pregnancy cohorts, and monozygotic twins discordant for asthma showing concordant FOXP3/IFN-Ξ³ methylation differences with T-cell function β a design that holds genetics constant .
The authors flag the core inferential gap: methylation measured in blood or buccal cells may not reflect airway tissue biology, and cell-type mixture confounds whole-blood methylation. Less acknowledged: the methyl-donor mouse diet finding runs counter to the human folic-acid literature, which the review admits is "mixed" across five cited cohorts β a genuine tension left unresolved rather than interrogated. The hygiene-hypothesis integration via prenatal Acinetobacter lwoffii exposure protecting offspring through preserved H4 acetylation at Ifng is intriguing but rests on a single animal study. As a narrative review tied to a 2013 ERS course, it carries selection bias risk; no systematic search strategy is reported, and the field has since moved substantially toward large EWAS with cell-type deconvolution, which this snapshot predates.
Confidence in the animal causal chain: high. Confidence that human methylation marks cause or even reliably predict asthma as of the review's date: moderate at best. The claims would be falsified if sentinel-loci methylation (Alox12, FOXP3, iNOS) failed to replicate in adequately powered cohorts with lung-relevant tissues, or if methylation manipulation at these loci altered Th1/Th2 balance in humans without changing asthma outcomes. Conversely, dose-response between verified prenatal exposure intensity and offspring methylation at asthma-relevant loci would substantially strengthen the causal reading.
