Core evidence chain. In a registered, single-center cohort (UMIN000015703), 30 CAD patients had significantly lower B. vulgatus and B. dorei abundance than 30 matched controls (confirmed by qPCR), plus significantly higher fecal LPS; Bacteroides abundance correlated negatively with fecal LPS (r=β0.227, P=0.0838 for genus; significant for the two species) .
Note: exact LPS values are in the original Figure 4C (medianΒ±IQR); the bar above schematizes the reported direction of effect, not precise magnitudes.
Causal mouse arm. ApoEβ/β mice gavaged with live B. vulgatus+B. dorei (2.5Γ10βΉ cfu, 5Γ/week, 10 weeks) showed reduced aortic-root and en face lesion area, less macrophage/CD4βΊ T-cell infiltration, lower plasma LPS and cytokines (IL-2, IL-4, IL-6, IL-17A, IFN-Ξ³, TNF-Ξ±), downregulated LpxA/LpxD, higher ZO-1, and more Tregs β with heat-killed bacteria ineffective, supporting live-microbe causality .
Critical caveats (authors partly acknowledged). 16S rRNA sequencing cannot resolve B. vulgatus vs B. dorei ; statin use differed drastically (90% CAD vs 40% controls) and 100% of CAD patients were on antiplatelet agents β a major confounder the paper does not fully address ; LAL assay measures total endotoxin, not immunogenicity; controls lacked angiography. Later work shows B. dorei effects are highly strain- and context-specific (e.g., pro-inflammatory metabolites induced by tetracyclines ), cautioning against naive probiotic generalization. Interestingly, B. dorei also protects against influenza via bile-acid/TGR5 signaling . What would falsify: multi-center metagenomics showing no species depletion, or human RCTs showing no lesion/endpoint benefit.
