Fernández-Borkel et al. (Universidad de Las Palmas de Gran Canaria, published July 16, 2025 in ACS Pharmacology & Translational Science) ask a genuinely unresolved question: are psilocybin's antidepressant effects in treatment-resistant depression (TRD) dependent on the conscious psychedelic experience itself, or driven purely by neurobiology? Their proposed randomized design dissociates these via three arms: (P) 25 mg oral psilocybin + guided integration therapy; (P+A) same dose under propofol-induced general anesthesia (4–6 h unconsciousness); (X+A) propofol + saline placebo . Outcomes: BDI/STAI clinical scores plus resting-state fMRI network analyses—Compact Box-Burning fractal dimension, Higuchi fractal dimension, and Lempel-Ziv Complexity—targeting DMN, SN, and frontoparietal networks .
Note: n=3 per arm (n=9 total); the figure illustrates the asymmetry of design elements—only the conscious arm receives integration therapy, a confound the authors themselves flag .
Strengths: The design directly tests a causal hypothesis (Yaden & Griffiths' claim that subjective effects are necessary) with a stronger manipulation than the risperidone-antagonist approach, since anesthesia fully suppresses consciousness rather than merely attenuating 5-HT2A signaling . The X+A arm smartly addresses psychedelic-trial unblinding, and propofol is rationally chosen: GABAergic mechanism without serotonergic interaction, well-characterized neural signatures distinct from psychedelic entropy profiles . No funding or competing interests are declared.
Weaknesses: (1) n=9 is explicitly not powered for hypothesis testing—only trend/feasibility detection . (2) The P vs P+A comparison conflates consciousness with integration therapy—the paper acknowledges this, but it guts the causal interpretation of the headline hypothesis. (3) Only P+A vs X+A are truly double-blind; the P group is inherently unblinded, re-exposing the expectancy problem the design claims to solve. (4) Propofol itself drastically alters entropy/complexity measures, complicating any fMRI comparison to conscious psilocybin states. (5) Maintaining 4–6 h of propofol anesthesia carries real anesthetic risk in a depressed population and ethical complexity the authors only partially address. (6) No empirical results exist—this is purely a protocol/hypothesis paper, so all claims remain speculative pending execution.
Falsification logic is well-specified: if P+A matches P clinically and neurobiologically, consciousness is not necessary; if only P improves, the authors' hypothesis gains support—though the therapy confound would still prevent attributing the difference to consciousness per se .
Bottom line (confidence: moderate-high): A conceptually novel, philosophically ambitious protocol with honest self-critique, but the integration-therapy confound and n=9 mean even successful execution would yield only weak causal evidence. Adding a fourth arm (placebo + integration therapy) is the single most important design fix. Falsification would come from any adequately powered trial showing equivalent P+A and P outcomes with matched post-session support.
