     Quick Explanation

The 2002 Neurology paper by Gydesen et al. defines FTD-3 by characterizing a large Danish Jutland pedigree (22 affected, autosomal dominant, linked to pericentromeric chromosome 3) with global cortical involvement, widespread rCBF reduction, and non-specific histopathology lacking tau inclusions, distinguishing it from FTDP-17 . The causative gene (CHMP2B) was only identified later .

     Long Explanation

    What the paper reports

Gydesen et al. present 17 years of data on a single large Jutland pedigree: 22 affected individuals across three generations, autosomal dominant inheritance with high penetrance, mean onset 57 years (range 46–65), mean disease duration 8 years, and death typically from pneumonia or intercurrent illness . The phenotype begins with personality change, disinhibition, and hyperorality; dyscalculia appears early; late stages bring pyramidal, extrapyramidal, and myoclonic features. Five autopsies uniformly showed neuronal loss and gliosis without plaques, tangles, or characteristic inclusionsβ€”criterion-defining for the "dementia lacking distinctive histopathology" category.

The key quantitative evidence: global hypoperfusion

The most distinctive finding is O-15-water PET in two patients. Even Case III-22 (age 64, MMSE 24β€”mild disease) showed pronounced rCBF deficits across all secondary cortices, while primary visual cortex, thalamus, basal ganglia, and cerebellum were spared. Because cerebellar flow was normal, the authors argue the cortical deficit reflects metabolic decline, not a vascular process. This global pattern contrasts with the frontal-only rCBF reduction reported for FTDP-17 .

Note: hemisphere values were averaged only for display; the paper reports right and left separately and did not publish an unaffected control mean, so "deficit" is defined relative to normative probability maps, not shown numerically.

How the field resolved the open question

The paper's central limitationβ€”no identified causative mutationβ€”was resolved post hoc. A 2003 follow-up found sparse tau filament deposits (60/64/68 kDa bands, AD-like) in the same family's frontal cortex without beta-amyloid, in cases II-12, III-13, and III-17 β€”partially revising the 2002 claim of tau-negativity. In 2008, a truncating splice-site mutation (c.532-1G>C) in CHMP2B, an endosomal sorting gene on chromosome 3, was identified in a previously unknown branch of the family , confirming FTD-3 as a genuine genetic entity and implicating the endolysosomal/ESCRT pathway in FTD pathogenesisβ€”a mechanism the 2002 paper could not have anticipated.

    Critical assessment

    • Strengths: exceptional longitudinal pedigree ascertainment (17 years), multi-modal characterization, and honest self-correctionβ€”the authors explicitly retract their earlier anticipation finding after two new paternal-transmission cases showed later onset than their fathers.
    • Blind spots: n=1 family severely limits generalizability; only one patient was neuropsychologically tested early; all patients with the late motor syndrome had neuroleptic exposure, so drug contribution to the extrapyramidal picture cannot be excluded (the authors note this but do not resolve it); white matter myelin loss may be secondary or coincidental small-vessel disease.
    • Self-correction needed later: the "lacks distinctive histopathology" framing was revised by the 2003 tau-filament data, illustrating how classification categories of the era were unstable.
    • The chromosome 9p ALS-FTD locus, cited in the paper as a comparator, was independently pursued in other families with multipoint LOD up to 7.22 , validating the paper's three-way genetic taxonomy (FTDP-17 / FTD-3 / MND-dementia) as a prescient framework.

What would change this assessment: evidence that the widespread cortical hypoperfusion in early disease reflects a comorbid process rather than CHMP2B-related pathology, or that non-Danish CHMP2B families show a materially different phenotype, would weaken the paper's central genotype-phenotype claim.

     BGPT Paper Review

    Study Novelty

    90%

For 2002, establishing a novel chromosome 3-linked FTD locus with full clinical-pathologic characterization was groundbreaking; CHMP2B confirmation came only in 2008.

    Scientific Quality

    80%

Rigorous multimodal characterization with honest retraction of the anticipation claim; weaknesses are single-family design, only two PET scans, and minimal early-stage neuropsychology.

    Study Generality

    50%

Findings are pedigree-specific; the genetic taxonomy framework generalizes, but phenotype details may not transfer to other CHMP2B or FTD cohorts.

    Study Usefulness

    80%

Anchored FTD genetic classification, enabled later CHMP2B mutation screening in Danish FTD, and provided a reference phenotype for familial FTD differential diagnosis.

    Study Reproducibility

    60%

Methods (PET protocol, histology panel) are described, but the family is unique and largely historical; independent replication of the full phenotype is essentially impossible.

    Explanatory Depth

    60%

Deep descriptive phenotyping, but no mechanistic explanation was possible pre-gene-identification; causal pathobiology awaited CHMP2B discovery.

     Hypothesis Graveyard

Anticipation with paternal transmission: the authors' own new cases (later paternal onset than fathers) falsified their 1995 claim, an unusually clean self-refutation.

FTD-3 as a pure tauopathy: the 2003 finding of sparse tau filaments revised the 'lacks distinctive histopathology' label, but tau is secondary, not causal.
