The hypothesis is a forward-looking testable claim, not an established result. The available evidence supports plausibility but is entirely computational plus indirect phenotypic correlation.
Supporting evidence. Six previously uncharacterized SMR-like transporters (uSMR1-uSMR6) were identified in disinfectant-tolerant S. nepalensis SDH.B1/B3 (uSMR1-4) and S. haemolyticus SDH.B7 (uSMR5-6) recovered from Vietnamese hospital surfaces. AlphaFold2 multimer predictions show all three putative heterodimer pairs (uSMR1/2, uSMR3/4, uSMR5/6) closely resemble the known quaternary-ammonium efflux transporter YkkC/YkkD, with TM-scores 0.87-0.96 and RMSD 0.77-2.23 Γ . Contextually, the isolates showed elevated BKC MICs (8-16 Β΅g/mL, ~4-8Γ above S. aureus ATCC 25923) and strong efflux activity (nicardipine-induced EtBr accumulation, up to 1.7-fold AUC increase) β but these phenotypes are confounded by the known plasmid-borne qacG and qacA pumps, which are equally plausible causal candidates .
Blindspots and confounders. The authors themselves state that functional assignment of the uSMRs is computational only, and that actual transport activity, substrate spectra, and in vivo oligomeric states await empirical verification . BKC-tolerant efflux phenotype could equally be attributed to qacG/qacA; genotype-phenotype discordances exist (e.g., tet(K)-positive SDH.B3 remained susceptible). uSMR5/uSMR6 occur in S. haemolyticus, not S. nepalensis, so the hypothesis partially exceeds its named scope. Structural similarity to YkkC/YkkD does not guarantee BKC efflux β SMR family members transport diverse substrates, and AlphaFold2 accuracy for membrane protein ionization states and lipophilic substrate binding is limited.
The cleanest falsification path: (1) Clone uSMR1-uSMR6 gene pairs under inducible promoters in a clean E. coli or S. aureus RN4220 background lacking native Qac pumps; (2) measure BKC MIC and growth curves vs empty-vector and positive-control (qacG) hosts; (3) quantify EtBr efflux with/without nicardipine; (4) complement with targeted uSMR deletion in SDH.B1 and MIC restoration on complementation. A null result (no MIC shift β₯2-fold across three biological replicates) would falsify the hypothesis per the study's own stated criteria . Confidence in the verdict (unproven but plausible) is moderate; confidence in uSMRs being functional BKC pumps is currently low and should not be overstated.
