The review's central claim β that mismatch-repair deficiency, via its enormous neoantigen load, is the dominant determinant of checkpoint-inhibitor response in CRC β was prescient and has been robustly validated. The pembrolizumab trial data it drew on reported a 40% immune-related ORR and 78% 20-week irPFS in dMMR CRC versus 0% ORR and 2.2-month median PFS in pMMR CRC, with dMMR tumors carrying ~1782 somatic mutations versus ~73 in pMMR (P=0.007). The review's coverage of Immunoscore, T-cell infiltration (CD45RO+ CD8+ density predicting recurrence beyond TNM stage), and immune-escape mechanisms (Tregs, WNT/TGF-Ξ²-associated exclusion) was accurate for its time and matched subsequent consensus.

Writing in 2017, the review could not anticipate several developments: (1) checkpoint inhibitor doublets now achieve ~55% ORR in MSI-H CRC β better than the ~40% monotherapy figure the review emphasized; (2) resistance mechanisms (Ξ²2M loss, JAK/STAT defects, antigen-presentation failure) were only emerging; and (3) the gut microbiome as a modulator of ICI response in MSS CRC was not yet a major research axis, though it is now recognized that microbiome effects are largely preclinical and no microbial biomarker is yet validated in MSI-H/dMMR CRC. The review's most serious gap is that it offers no strategy for the ~95% of CRC that is MSS β a limitation later systematic reviews identify as the field's central unsolved problem, with T-cell exhaustion reversal in MSS CRC still showing only early-phase signals.
Confidence: moderate-to-high. The core MSI-H/dMMR biomarker claim rests on small (n=10β18 per cohort), non-randomized single-arm data in the source trial, but has since been replicated across independent agents and combination regimens, making reversal unlikely. The Immunoscore prognostic claims rest on a 1336-patient validation and remain strong. The review's narrative-review format means it cannot disentangle selection bias in the early trials it summarizes, and its optimistic framing of then-ongoing prospective Immunoscore trials was subsequently only partially borne out.
