This systematic review by Huang et al. searched PubMed (inception to July 2015) for studies on microRNAs in psoriasis, with 3 independent investigators, no language or study-type restrictions, and coverage of skin, blood/PBMC, and hair samples .
Strengths: The review organizes a diffuse literature into mechanistic frameworks — notably the miR-31/NF-kB/ppp6c feedback loop and the miR-21/STAT3 network — and links miR-146a rs2910164 genotype (OR 1.38; 95% CI 1.06–1.80) to keratinocyte hyperproliferation via reduced EGFR repression .
Limitations & skepticism: The review was never registered in PROSPERO, lacks a PRISMA flow diagram, quality scoring of included studies, and meta-analysis; search coverage ends mid-2015, so the now-extensive integrated miRNA/mRNA profiling era is absent. Later work profiles 28 monotonic miRNAs across non-lesional→peri-lesional→lesional skin with 3,735 inverse miRNA–mRNA correlations (n=14 patients), substantially deepening the picture . Most included primary studies are small, single-population (largely Han Chinese and European cohorts), and correlative — conflating association with causation. Publication bias favoring dysregulated miRNAs is likely. Contradiction blindspot: miR-424 is reported downregulated in lesional skin yet upregulated in blood and hair shaft, which the review does not reconcile.
Verdict (moderate confidence): A useful, well-organized narrative map of pre-2015 psoriasis miRNA biology with genuine mechanistic synthesis, but methodologically weak as a systematic review; its biomarker claims remain unvalidated prospectively. Evidence that would change this verdict: replicated negative studies for miR-223/miR-143 PASI correlation, or registered meta-analyses confirming diagnostic utility.
