Using TCGA reverse phase protein array (RPPA) data, the authors trained an elastic-net penalized Cox model (glmnet, Ξ± = 0.5) on 102 primary untreated GBMs, selecting 13 proteins, and locked the Ξ² coefficients to score an independent 101-patient validation set. High-risk patients had markedly shorter median survival: 22.2 vs 12.0 months (training, p < 0.0001) and 16.9 vs 10.6 months (validation, p = 0.0009), with C-indices of 0.63 and 0.60. PROTGLIO remained significant in multivariable Cox models (p = 0.008 in both sets) alongside age, KPS, resection extent, TMZ, and MGMT methylation .
TGM2 carries the largest positive coefficient (0.54, higher abundance = higher risk); total Src and p-Src are protective (Ξ² = -0.20 and -0.17). Cox-2 and ANXA1 appeared in 9/10 repeated model-generation runs, with IGFBP2, p-CHEK1, and TGM2 in 7/10, and functional enrichment pointed to inflammatory/immune response pathways .
The LGG corroboration is indirect: a separate 12-protein IDHwt model (42 patients, only 21 per split) shared five proteins (AMPK1, ANXA1, CHEK1, IGFBP2, SRC) with the GBM model, but Cox-2 was not measured on the LGG array, so the PROTGLIO signature itself could not be computationally re-validated . Critically, the 'validation' set is a random split of the same 203 TCGA tumorsβnot a fully external cohortβand repeated 2-fold CV yielded 37 different significant proteins across runs, so the exact 13-protein composition is unstable even if the score's predictive direction is reproducible. GCIMP/IDH1 could not enter the multivariable model for lack of degrees of freedom. The authors themselves call for retrospective and prospective proteomics studies before clinical use.
Confidence: moderateβthe internal validation is statistically solid and the biology is plausible, but single-cohort dependence, signature instability across resampling, modest C-indices (~0.6), and no true external replication temper clinical translation claims.
