HLA means Human Leukocyte Antigen β named because the molecules were first identified on white blood cells (leukocytes) and act as antigens in transplant reactions. Encoded within the MHC region on chromosome 6, HLA genes are the most polymorphic in the human genome. The HLA protein family splits into classical class Ia (HLA-A, -B, -C, presenting intracellular peptides to CD8+ T cells), class II (HLA-DR, -DQ, -DP, presenting extracellular peptides to CD4+ T cells), and non-classical class Ib molecules (HLA-E, -F, -G) with specialized immune-regulatory roles.
Interactive structure of an HLA class I molecule (HLA-A2, PDB 1YCR): the heavy chain (cartoon) presents a bound peptide (stick) to T cell receptors.
HLA class I molecules bind short endogenous peptides. Even platelets carry HLA-A2 loaded with peptides from megakaryocyte-lineage proteins (e.g., the platelet-specific GPIX-derived ligand FLLWATAEA), although these molecules fail to stimulate direct allo-cytotoxic T cells and appear to be vestigial rather than functional antigen presenters . Accessory HLA molecules such as HLA-DO modulate class II peptide loading in B cells, fine-tuning antigen presentation .
Specific HLA alleles drive disease susceptibility. In a Japanese case-control study (120 patients vs. 817 controls), HLA-A*02:06:01 was the primary susceptibility allele for cold medicine-related Stevens-Johnson syndrome with severe ocular complications (OR = 5.46, p = 1.15 Γ 10β»ΒΉβΈ), while protective alleles (e.g., A*24:02:01, OR = 0.46) were also identified . Non-classical HLA molecules regulate immunity: HLA-G suppresses antigen-specific cytotoxic T lymphocyte killing , and HLA-E is highly expressed in gametogenic cells, potentially protecting germ cells from autoimmunity .
The term itself is unambiguous, but functional interpretations carry caveats: platelet HLA findings derive from a single HLA-A2 allele focus, in vitro conditions, and pooled donor samples with limited raw-data transparency . The SJS association was population-specific (Japanese) and subject to linkage-disequilibrium confounding, so allele-disease mapping may not generalize across ancestries. What would change these conclusions: replication of allele associations in independent non-Japanese cohorts, and in vivo demonstration that platelet HLA can stimulate alloimmunity.
