In male SOD1G93A mice treated from 13 weeks (post-onset), UA-30 100 mg/kg prolonged mean lifespan (171.5 Β± 8.7 vs 161.9 Β± 8.1 days, P=0.027; +8.5 days median survival, n=14 vs 17 vehicle; riluzole n=13, P=0.107 vs UA-30) . High-dose UA-30 beat riluzole numerically on most motor endpoints (e.g., +64.5% male wire-hang, +14.9% rotarod) and preserved ~20% more spinal motor neurons, though no sex Γ treatment interaction was detected (all P>0.32). Female survival benefit was not statistically significant β an honest but underpowered comparison.
For context, ALS remains largely untreatable; parallel mechanism-first strategies (cGAS inhibition in TDP-43 models and TDP-43 function restoration via AAV ) show far larger effects, underscoring that an 8.5-day median gain is modest for SOD1G93A.
The causal chain β UA-30 binds RalA (CETSA/SPR) β βRalA-GTP β βp-ERK β FOXO3a deubiquitination/stabilization β βnuclear FOXO3a β βPINK1/Parkin mitophagy β restored ETC complexes and ATP β is supported at each step and logically inverted twice: RalAG23V overexpression and Mdivi-1 each abolished UA-30's mitochondrial and behavioral benefits . RalA inhibition is mechanistically plausible: constitutively active RalA drives mitochondrial fission in obese adipocytes, and RalA relocates to depolarized mitochondria. Weak links the authors partially acknowledged: Mdivi-1 acts on Drp1 fission, not mitophagy directly, so it is an indirect, ambiguous inhibitor; Mdivi-1-alone controls (WT behavior, SOD1G93A survival) were not run; and ~100% changes in PINK1 protein vs only mild transcript shifts imply an unexplained post-transcriptional layer the FOXO3a model does not cover.
Bottom line: a well-constructed preclinical package with unusually complete reversibility controls, but translational weight rests on model fidelity and unproven RalA-inhibition safety. No treatments can be recommended on this evidence.
