Pickup, Mouw & Weaver argue the ECM is not passive scaffold but an active signaling platform whose biomechanical and biochemical properties modulate all cancer hallmarks. The review integrates 134 references showing matrix stiffness, collagen crosslinking, and MMP activity regulate proliferation, apoptosis resistance, angiogenesis, invasion, immune evasion, and metabolic reprogramming through conserved signaling nodes.
Blue: ECM properties; Orange: signaling pathways; Green: hallmarks. Compiled from mechanism descriptions in Pickup et al.
The authors acknowledge that ECM influence cannot be broadly applied to all cancer types: stromal fibroblast ablation in pancreatic tumors was inhibitory to progression, contrary to breast cancer data where increased ECM deposition enhanced progression. Whether matrix stiffness is causative or correlative remains unresolved. Shh inhibitors that showed preclinical promise for pancreatic desmoplasia failed in clinical trials, attributed to genetic differences between models and patients or systemic effects on multiple cell types.
Key unknowns: (1) Which cellular source (epithelial vs stromal) dominates ECM remodeling in each tumor type; (2) Optimal timing for ECM-targeted intervention; (3) How ECM heterogeneity within a single tumor affects therapeutic response. The review proposes transgenic models targeting ECM expression from specific cell populations as the path forward.
