Bai et al. treated primary neonatal mouse microglia with LPS (100 ng/mL) or IL-4 (10 ng/mL) for 24 h, verified polarization by qPCR (CD86/TNF-Ξ±/iNOS vs. Arg/CD206/YM-1) and Western blot (iNOS vs. Arg), then isolated exosomes by differential ultracentrifugation. Vesicles were ~100 nm cup-shaped by TEM, 30β150 nm by NTA, positive for Alix, HSP70, Flotillin-1, and CD9, and internalized by Dil-stained uptake into neuronal cytoplasm .
Functionally, after oxygen-glucose deprivation/reoxygenation, normalized neuron survival fell to 0.497 Β± 0.029 with M0 exosomes, dropped further to 0.335 Β± 0.027 with M1 exosomes, and rose to 0.652 Β± 0.020 with M2 exosomes; TUNEL and Calcein AM/PI staining agreed. In tMCAO mice given 100 ΞΌg exosomes by tail vein for 3 days, M1 exosomes enlarged infarcts while M2 exosomes reduced infarct volume and improved mNSS, grip strength, and rotarod performance . This mirrors earlier work where M2 microglial exosomal miR-124 protects against ischemia-reperfusion injury .
Proteomically, TMT-LC-MS/MS (MaxQuant) identified 1498 proteins and quantified 1129: M1 vs M0 exosomes had 111 up/91 down DEPs, M2 vs M0 had 126 up/105 down, and M2 vs M1 had 89 up/96 down. GO enrichment assigned inflammatory response, neutrophil chemotaxis, and complement activation to M1 exosomes vs. protein transport and anti-apoptotic growth regulation to M2 exosomes; KEGG linked M1 to complement/coagulation, apoptosis, and NF-ΞΊB and M2 to glycolysis, antigen presentation, and MAPK. PPI hubs included IL-6, SAA3, CCL5, CCL9, C3, CFB, SRGN (M1-side) and SPP1 (downregulated in both activated states, contrasting its synaptophagy role in Alzheimer's models) .
Strengths: use of primary microglia rather than BV2 cells (which only partially replicate primary microglial behavior ); both in vitro and in vivo functional assays; adequate exosome characterization; honest, explicit limitations section acknowledging unvalidated cargo proteins, simplified M1/M2 induction, acute-phase-only tMCAO observation, and missing co-treatment experiments.
Weaknesses: no functional validation of any individual exosomal protein β the mechanistic link between the proteome and phenotype is correlational; animal and culture sample sizes are not stated in the methods; ultracentrifugation alone risks co-isolation of non-exosomal vesicles and protein aggregates (no density gradient or SEC control); exosome dose and uptake kinetics in vivo are not dose-optimized; the M1/M2 dichotomy itself is contested as an oversimplification of microglial states; only male mice were used, ignoring sex effects documented in other microglial contexts; no public data deposition (proteomics available only "on reasonable request"), limiting reproducibility; western blot exosome markers lack a negative control (e.g., calnexin). The directional concordance across in vitro and in vivo assays is the strongest evidence; the proteomic interpretation is hypothesis-generating.
Context: related state-specific EV proteomics exists in BV2 cells , so the conceptual novelty is incremental; the primary-microglia setting and stroke-relevant functional pairing are the real contributions. Confidence in the qualitative direction (M1-EXO harmful, M2-EXO protective in acute mouse ischemia) is moderate; confidence in specific protein-level mechanisms is low pending knockdown/rescue experiments. Data that would change this conclusion: independent replication with size-exclusion-purified vesicles, sex-balanced cohorts, and single-protein perturbations (e.g., SRGN or C3 depletion) failing to alter the observed effects.
