The review synthesizes the clinical development program for nirsevimab, from preclinical potency (β50-fold greater neutralization than palivizumab across 102 clinical isolates) through pivotal trials and postlicensure surveillance. Its core quantitative claims are directly traceable to the underlying trials: MELODY reported 74.5% efficacy (95% CI, 49.6β87.1) against medically attended RSV-LRTI, and the pragmatic HARMONIE trial reported 83.2% (95% CI, 67.8β92.0) against RSV-LRTI hospitalization. Real-world meta-analysis of 32 case-control studies found OR 0.17 (95% CI 0.12β0.23) for RSV hospitalization and OR 0.19 for ICU admission, though length of stay did not differ significantly.
Resistance is the review's most important caveat, and it is handled with appropriate nuance. In 2024/25 French breakthrough infections, 12.5% of RSV-B but only 1.0% of RSV-A variants showed reduced susceptibility, with residue 208 central to RSV-B escape; no resistant variants were found in nirsevimab-naive infants. Deep mutational scanning explains this subtype asymmetry: bivalent IgG binding buffers many mutations in RSV-A but less so in RSV-B.
Comparative positioning is observational, not randomized. The claim that nirsevimab outperforms maternal RSVpreF vaccination rests on a single matched French cohort (42,560 infants; HR 0.74 for RSV-LRTI hospitalization, HR 0.58 for PICU admission) β credible but subject to confounding by gestational timing and placental transfer.
Conflicts of interest warrant weight adjustment. The senior author discloses PI roles in trials for numerous manufacturers including Sanofi/AstraZeneca, honoraria, and advisory roles with both nirsevimab sponsors; the co-author reports Sanofi travel support. The review is also explicitly non-systematic β no search strategy, no risk-of-bias grading β and its conclusion that implementation cost in LMICs depends on sub-$25 dosing prices (vs. ~$495 US list price) is stated honestly but unresolved.
Confidence and falsifiability: The efficacy/effectiveness conclusions rest on strong randomized and consistent observational evidence and are unlikely to be overturned. They would change if multi-season postlicensure effectiveness declined substantially or if RSV-B resistance variants rose beyond low frequency among hospitalizations. Data missing from this review: long-term (β₯3 season) impact, LMIC real-world effectiveness, and head-to-head trials vs maternal vaccination or clesrovimab.
