Vallianou, Evangelopoulos and Kazazis (Review of Diabetic Studies, 2013) argue that metformin activates the LKB1/AMPK axis, inhibits mTORC1, kills cancer stem cells, and lowers cancer incidence/mortality in type 2 diabetes. The mechanistic half of the review is faithful to strong primary literature: AMPK activation by LKB1, TORC2 phosphorylation, and AMPK-mediated mTORC1 inhibition via TSC2/raptor phosphorylation are accurately summarized .
The review is candid about the dose problem: most in vitro anti-proliferative effects occur at concentrations "at least 10-fold higher" than peak clinical plasma levels . Independent quantification confirms this gap is large: breast cancer MCF-7 cells show 59% inhibition and 62% G1 arrest at 20 mM metformin , while cervical cancer HeLa/SiHa IC50 values are 19-25 mM , versus a realistic clinical compartment of ~1-2 mM even in gut, and a terminal half-life of ~52 h only in accumulation states .
The epidemiological half is where the review is weakest. It cites favorable cohort signals β adjusted OR 0.77 for any cancer (Tayside), cancer mortality 6.3 vs 9.7 per 1000 person-years vs sulfonylureas, OR 0.43 for cancer mortality (Dutch cohort), monotherapy OR 0.90 vs untreated (UK) β but also contains site-specific null or adverse results within its own cited evidence: bladder OR 0.99 and pancreatic OR 1.26 in a 191,233-patient US cohort . Vallianou et al. cite Suissa and Azoulay but do not integrate its central finding: time-related biases (immortal time, time-lag, recall) plausibly generate the entire protective signal in these designs .
Subsequent human data were sobering: a window-of-opportunity trial found only a 3-point Ki67 drop (36.5 to 33.5%, p=0.016) with no control arm , and the MA.32 trial later reported no significant disease-free survival benefit β a falsification the review's own authors anticipated by acknowledging that "large clinical trials" might not corroborate favorable effects.
Correct mechanism sketch, honest about the dose gap, but the epidemiological synthesis reads as confirmation rather than adversarial testing. Confidence in this assessment: high for the dose-gap and bias critique (directly quantified); moderate for translational predictions.
