Leng et al. sequenced peripheral blood from 20 sepsis patients and 10 healthy controls, identifying 948 differentially expressed genes (669 up, 279 down) under stringent thresholds (FDR < 0.01, |FC| β₯ 4). PPI hub analysis via STRING/cytoHubba MCC ranked ELANE, CTSG, MPO, BPI, and AZU1 as central nodes β all neutrophil granule genes, which suggests the 'core gene' signal largely reflects neutrophil activation rather than macrophage-specific biology .
Meta-analysis across GSE28750, GSE67652, GSE69528, and GSE95233 confirmed upregulation, though reported heterogeneity was very high (IΒ² = 86%), and survival analysis in GSE65682 (n=802) associated high ELANE with lower 28-day survival. scRNA-seq of 5 donors localized ELANE to macrophage clusters (3 and 5) .
Cellular localization tension. ELANE is classically a neutrophil primary granule protease; the scRNA-seq macrophage assignment (clusters 3 and 5) rests on S100A12, CD300E, and CD14 markers in just 5 donors and may reflect low-level neutrophil contamination or immature myeloid cells. Whole-blood RNA-seq cannot disentangle these compartments. Independent neutrophil multi-omics work shows ELANE is a sex-biased primary granule gene in neutrophils , and elevated ELANE/MPO/CD177 signatures in acute-on-chronic liver failure were interpreted as neutrophil-driven .
Functional causality is thin. The THP-1/PMA/LPS system with siELANE-3 shows reduced CD86+ fraction and lower IL-1Ξ²/TNF-Ξ± after knockdown, but THP-1 is a monocytic leukemia line and, critically, ELANE is not typically expressed in monocyte-derived macrophages β the baseline expression the siRNA targets is questionable, and no rescue (re-expression of siRNA-resistant ELANE) was performed. No primary human monocyte-derived macrophages, no ELANE-knockout mouse sepsis model, and no protein-level ELANE measurement in supernatants were included β limitations the authors themselves acknowledge .
Confounders unaddressed. Flow cytometry tables report only group means of small replicate numbers without error bars or statistical test details; the M1/M2 gating logic in Fig. 9 (Q3 quadrants) is unconventional. Sepsis cohort heterogeneity (Sepsis-2 vs Sepsis-3 definitions, pediatric/adult mixing in GSE67652) inflates the meta-analysis IΒ². Elsewhere, ELANE hub genes appear repeatedly in unrelated inflammatory contexts (osteomyelitis network pharmacology AUC 0.976 ), suggesting ELANE is a generic granulocyte-activation marker rather than a sepsis-specific driver.
The causal claim would strengthen with: ELANE protein quantification in sepsis plasma, primary MDM experiments with recombinant neutrophil elastase add-back, and survival benefit of ELANE inhibition (e.g., sivelestat analogs) in CLP mice. Falsification would occur if ELANE knockdown fails to reduce M1 markers in primary macrophages or if ELANE-upregulation disappears when neutrophils are depleted from the analysis.
