Reshkova's narrative review compiles prior meta-analytic comparisons of serum biomarkers between ILD-RA and RA without ILD. Reported pooled differences favoring ILD-RA include RF (MD 159.82; 95% CI 107.06β212.58; IΒ² = 93.33%), anti-CCP (MD 54.68; 95% CI 28.47β80.88; IΒ² = 87.50%), KL-6 (MD 590.25; IΒ² = 94.60%), and SP-D (MD 78.59; IΒ² = 97.77%) . Severity claims rest on small pooled samples: KL-6 negatively correlated with FVC (r β0.26) and DLCO (r β0.33) in 237 ILD-RA patients, MMP-7 with DLCO in 243, and HE4 with FVC% in 94 . Prognostically, KL-6 predicted poor outcome (HR 3.01; 95% CI 1.57β5.76) from only 146 patients across two studies . PLR (P = 0.64) and CA-125/CA-153 showed no difference, a useful negative finding . The review also highlights regional heterogeneity β ESR and anti-CCP differences seen only in Asia/Africa, RF elevated across all regions .
Strengths: Broad coverage (13 countries; 98 articles screened for quality), the bidirectional RAβILD causality discussion (mucosal citrullination, PAD2, IgA anti-CCP) is well-framed , and the first randomized trial of pirfenidone in ILD-RA is correctly emphasized . Weaknesses: no original data, no stated search strategy, inclusion criteria, or formal quality grading despite claiming "98 articles, 83 high quality"; treatment sections list drugs without effect sizes or evidence grading; several reference entries are garbled (Medscape, inconsistent DOIs), and the text contains internal errors (e.g., calling NSIP prognosis "poor" in one sentence and "relatively good" in the next; NSIP abbreviated as "NUIP"; IPF conflated with UIP). The pirfenidone trial had only 123 patients and a null primary endpoint , which the review's optimistic framing underplays. Novel antigens like EXOSC4 (AUC 0.809; 0.89 combined) are absent from the review despite emerging validation value . Notably, RF elevation with IΒ² >93% offers little discriminative specificity for an individual patient β a translational gap the review acknowledges only partially. What would change these conclusions: prospective cohorts with predefined cutoffs and validation showing RF/KL-6 fail to discriminate or predict; larger, adequately powered RCTs reporting null antifibrotic benefit .
