This study functionally characterized 30 CYP2C19 variants (mostly rare, identified in 2,127 Chinese Han individuals) using recombinant enzymes in Sf21 insect-cell microsomes and a validated HPLC-FLD assay for citalopram N-demethylation
Key reported findings: 22/30 variants showed significantly altered activity. CYP2C19*29 (K28I) showed ~4.9-fold increased CLint (767.55 ± 492.31 vs 156.14 ± 8.87 µL/min/nmol P450) and L16F ~1.5-fold; CYP2C19*3 and the novel 35FS frameshift variant were completely inactive. Eighteen variants showed reduced CLint (18–63% of wild-type)
Clinical anchor: The citalopram-relevant substrate-dependence is consistent with in vivo pharmacokinetics: Chinese CYP2C19 poor metabolizers showed ~23% higher citalopram AUC (2132.5 vs 1738.0 µg·h/L, P<0.01) and lower desmethylcitalopram exposure vs extensive metabolizers in a single-dose study of 13 healthy men
Critical appraisal: Strengths include systematic variant coverage, triple replicates, validated HPLC-FLD, and raw data availability. Weaknesses: (1) in vitro-only; no IVIVE performed; (2) substrate-dependence is real — the authors show *29 increased citalopram CLint ~5-fold but reduced fluoxetine metabolism to 43% of wild-type, so extrapolation to other drugs is unsafe; (3) only key amino-acid substitutions were built, not full haplotypes; (4) Sf21 expression lacks physiological cofactor context; (5) I331V carries 90.16% allele frequency yet reduced CLint ~15% — the clinical impact of such a common mild-effect variant is unresolved; (6) no correction for multiple comparisons across ~30 variants × multiple parameters.
Confidence note: The variant-level kinetic findings are well-supported within the in vitro system. Translation to clinical dosing remains inferred, not demonstrated — the authors themselves acknowledge this and call for IVIVE and clinical validation. Falsification would require in vivo genotype-phenotype studies showing carriers of these rare variants have normal citalopram clearance.
