Chen et al. survey nucleic-acid epigenetics from detection chemistry to clinical translation, covering the 5mC oxidation cascade (5hmC, 5fC, 5caC via TET/TDG/AID-APOBEC pathways), RNA marks (m6A via METTL3/METTL14/WTAP with FTO/ALKBH5 demethylases, m1A, pseudouridine, RNA 5mC/5hmC), and the emerging DNA 6mA mark in eukaryotes . The review's strongest asset is its chemical precision: it systematically links each mark's functional-group chemistry (redox potential, hydroxyl, aldehyde, glucosylation susceptibility) to the detection method built upon it.
The paper reports real detection-limit values for mass-spectrometry workflows: 0.06 fmol LOD for 5mC (cHILIC-in-source fragmentation-MS/MS), 0.056 fmol for 5hmC (LC-MS/MS/MS), and 363-fold sensitivity gains for 5fC via dansylhydrazine labelling .
These numbers justify the review's central methodological claim: chemical-labelling strategies transformed epigenetics from qualitative (TLC, antibodies) to quantitative, single-base, and single-cell resolution (nano-hmC-Seal at ~5 ng DNA, scAba-seq, nanopore discrimination of all five C5-cytosines) .
The disease section (cancer, T2D, obesity, Alzheimer's, Parkinson's) is broad but correlational. Notable reported findings include globally depleted 5hmC across kidney, breast and colorectal tumours, HIF3A methylation associated with BMI, and persistent methylation in the DCCT/EDIC T1D cohort supporting an epigenetic "metabolic memory" . The authors honestly flag that it remains unclear whether epigenetic changes are initiators or consequences of tumourigenesis β a commendable caveat, though the same causality caveat applies with equal force to the obesity, diabetes and neurodegeneration sections, where tissue specificity (blood vs. target tissue) and cellular heterogeneity are the dominant confounders. Contemporary epigenetic-epidemiology reviews similarly note that small methylation differences, tissue discordance, and multiple-testing burdens limit causal inference .
Bottom line: A high-quality, chemistry-anchored snapshot of the field as of 2016/2017. Its disease-to-biomarker narrative should be read as hypothesis-generating; what would disconfirm the clinical trajectory is failure of methylation biomarkers to replicate prospectively in large clinical cohorts β which the paper itself concedes has not yet happened at scale.
