The central paper analyzed 2,148 dementia-free Swedish community-dwelling adults (median age 72.2 years; 61.5% female; 381 with baseline MCI) over a mean 9.6-year follow-up using serum Simoa assays for AΞ²42/40, p-tau181, p-tau217, t-tau, NfL, and GFAP, with four-state multistate Markov modeling treating death as an absorbing state. Elevated NfL showed the strongest associations with MCI-to-dementia progression (HR 1.84, 95% CI 1.43β2.36 for all-cause dementia; HR 2.34, 95% CI 1.77β3.11 for AD dementia), followed by p-tau217 (HR 1.74 and 2.11, respectively). Combinations amplified risk: three elevated biomarkers yielded HR 2.22 (1.50β3.28) for all-cause and HR 3.71 (2.22β6.20) for AD dementia. Notably, no biomarker predicted normal-cognition-to-MCI transition, positioning the MCI stage as the actionable window for blood-based risk stratification .
A meta-analysis of 44 studies (9,343 participants) independently corroborates p-tau217 as the top-performing marker for MCI-to-AD conversion: ratio of means 1.86 (95% CI 1.53β2.27), diagnostic odds ratio 42 (95% CI 21β81), and AUC 0.93 with 91% sensitivity and 81% specificity. P-tau181 (DOR 15, AUC 0.86) and GFAP (DOR 12.71, AUC 0.84) follow; AΞ²42/40 was weaker (DOR 6, specificity only 49%) but complementary. Results varied by MCI subtype and assay platform . Narrative reviews converge on the same hierarchy, noting p-tau217's superiority and regulatory progress (e.g., Lumipulse pTau217/AΞ²42 ratio) while flagging inter-assay variability and absent universal cutoffs .
Translation gap. In a primary-care-oriented validation, plasma AΞ²42/40 reached only 70% sensitivity/73% specificity; negative predictive power exceeded 0.90 at 10β12% base rates, but positive predictive power was just ~0.22βinsufficient for stand-alone diagnosis, useful only as a triage rule-out .
Comorbidity confounding. Impaired kidney function (CKD stage 3) elevates AΞ²40 and NfL and alters amyloid-positivity prediction . The SNAC-K authors did adjust for comorbidities (CKD prevalence 33.4%), partially addressing this, but residual confounding cannot be excluded.
Population validity. In a race/ethnicity-representative midlife US cohort (n=4,340), no meaningful race/ethnicity differences in plasma biomarkers persisted after population weighting and comorbidity adjustmentβreassuring for threshold generalizability, though p-tau217 was not measured .
Structural caveats in the core paper. Serum (not plasma) measurements, baseline-only biomarker sampling, clinical dementia diagnoses without CSF/PET confirmation, SNAC-K-derived cutoffs limiting external generalizability, and substantial missing biomarker data with attrition bias. The null NCβMCI finding may reflect the long preclinical trajectoryβbiomarkers may rise decades before MCI onsetβrather than true non-predictiveness; a single baseline draw cannot test that.
Falsification path: independent cohorts showing no biomarker-transition associations, or failed replication of p-tau217/NfL HRs with standardized plasma assays, would overturn the core conclusion. What would strengthen it: replicated cut-offs across platforms, longitudinal biomarker trajectories, and autopsy- or PET-confirmed endpoints.
